Information and Assistance (I&A) Frequently Asked Questions

Call Council on Aging Silicon Valley Health Insurance Counseling and Advocacy Program (HICAP) at 1 (800) 434-0222. HICAP is a program offered by COASV to Medicare or soon-to-be Medicare beneficiaries and/or their caregivers on all aspects of Medicare and other senior-related health insurance needs. We also provide long term care insurance counseling services. Specially trained counselors are available to advise you Monday-Friday, 8am-5pm (excluding holidays).

Call In-Home Supportive Services (IHSS) at (408) 975-4899 and request to speak to your IHSS social worker.

You can find answers for many of your questions by calling resources, such as Caring Connections, a comprehensive end-of-life resource organization at 1 (800) 658-8898 and Coda Alliance at (408) 267-3922 or their www.codaalliance.org where you can find End-of-life forms and instructions on filling them out.

A pamphlet entitled ‘Five Wishes’ is available by contacting Aging With Dignity, 1 (888) 5-WISHES, www.agingwithdignity.org; in it you can detail your wished should you become seriously ill.

Contact Eldercare Locator at 1 (800) 677-1116 or www.eldercare.gov can help you locate the Area Agency on Aging in the area your parents live, anywhere in the nation. Locating help for an older family member or friend is easier than ever before with the Eldercare Locator – Community Assistance for seniors.

You can locate your nearest Social Security office by using their online office locator and entering you zip code.

Transportation services are available throughout Santa Clara County. Council on Aging Silicon Valley's Information & Assistance Program can assist you with information to access these services. Call an COASV Information & Assistance Specialist at 1 (800) 510-2020.

For more information on public transit and discounts available to seniors and the disabled in the Santa Clara County, contact the Valley Transit Authority at (408) 321-2300 or www.vta.org.

Call Council on Aging Silicon Valley at 1 (800) 510-2020 to discuss the various types of housing options available in Santa Clara County.

Our State Long Term Care Ombudsman Program would be glad to investigate complaints. To reach them you may call (408) 944-0567 or 1 (800) 231-4024 to reach their 24-hour Crisis Line.

Call Council on Aging Silicon Valley and speak with a Case Manager to discuss Alzheimer’s disease and the respite and support programs that may be available to you and your mother. You may also contact the Alzheimer’s Association of Northern CA at 1 (800) 272-3900 and request educational material or information about support groups.

You can contact Council on Aging Silicon Valley and speak with a Case Manager. They can assess your mother’s situation and begin helping you implement services she needs now and will need in the future.

The Council on Aging Silicon Valley’s Health Insurance Counseling and Advocacy Program (HICAP) offers confidential consultation and advice for consumers interested in medical and long-term care insurance to help educate them in their decisions pertaining to medical benefit insurance plans, long-term care insurance, Medicare and Medi-Cal, and medical bill payments.

Access to HICAP is as simple as placing a phone call to COASV. A HICAP counselor will answer your questions and may schedule an appointment with you depending on your needs. Call our HICAP information line at 1 (800) 434-0222.

There are a number of grief resources available in Santa Clara County such as Centre for Living with Dying, (408) 553-6950, and Kara-Grief Support for Children and Adults, (650) 321-5272. Contact the Council on Aging Silicon Valley and speak to an Information & Assistance Specialist about other grief-related resources available in our area.

By calling resources, such as Heart of the Valley, Services for Seniors, (408) 241-1571, and the City of San Jose, Senior Companion Program, (408) 979-7904, you may be able to connect your friend to a Volunteer companion in her area. Contact Bay Area Legal Aid’s Legal Assistance Line at (408) 850-7066. Many times they can prolong or, in some cases, stop the eviction. Forms are also available from the Santa Clara County Self-Service Center website at www.scselfservice.org.

If you are worried about a senior’s welfare you can dial 3-1-1 or contact the police or sheriff’s department directly and request a well-being or welfare check. You can also call Adult Protective Services (APS) directly at (408) 975-4900.

Able People Foundation provides mobility equipment to qualifying individuals. Contact them by going to their www.ablepeoplefoundation.org or by calling them at (408) 263-8000.

Please call Council on Aging Silicon Valley at (408) 350-3246 and speak to a Nutrition Coordinator in the Meals on Wheels department.

Contact Council on Aging Silicon Valley’s Predatory Fair Lending Program at (408) 350-3229 and talk to a Fair Lending Specialist about options your parents may have.

Rebuilding Together Silicon Valley assists qualifying low-income homeowners of Santa Clara County with home modification that will ensure safety and independence. Contact them at (408) 578-9519.
