Council on Aging Silicon Valley’s Care Management provides customized care solutions to help eligible seniors age well at home. Additionally, Care Management supports family caregivers to ensure they receive support and services needed for the continued well-being of both caregiver and care receiver.

Our Multipurpose Senior Services Program (MSSP) assists persons who are Medi-cal eligible and over age 65 with support services needed to maintain independence and allow a senior to age well in their own homes.

Our Family Caregiver Support Program (FCSP) supports caregivers for older adults, age 60 and above. Care management services include information about available services and assistance in gaining access to those services. In addition to individual counseling, training and support group assistance may be provided.
